16 vs T will occur about 3.5% of the time. If you have trouble finding the rest, let me know I could spend the time looking it up when I get home.
The answer will vary based on number of decks. Professional Blackjack by Wong has an appendix of hand frequencies for 6 decks, you could look it up there. Or total up the frequencies of hands from the appendix in Blackjack Attack (or find cacarulo's frequency tables online and total it up there).
