KO count questions

I'm a newbie so bear with me. I have a couple of questions about KO count. For a six deck game the IRC is -20 and the key count is -4. Does this mean I should start betting larger bets only when I have reached -4 or above? If the count gets up to +5 etc, is that still good to bet? My second question is this: do you only start counting at the beginning of a new shoe? and does anything change after a shuffle? I know these are basic questions but I can't seem to find the answers. Thanks!
