The app is called poker income pro, definitely one of the best poker apps I have used. The chart is from a screenshot of an excel spreadsheet I made for blackjack. I tried to post the spreadsheet a year back, but because of my customization, it's a fairly large file and it was too big to be accepted as an attachment
